Match Report Forms

Please ensure match report forms are completed correctly, all the relevant information is entered and stapled together before posting them to me. Ensure when entering the players onto the team sheet, that the Forename and Surname is entered using BLOCK CAPITALS only (Upper case)

Before posting to me, check to see if you have completed the following

Date of match, Division to which fixture applies, Indicate whether you are the Home or Away team (delete as appropriate), Enter the Home and Away team where indicated, Enter a mark for the referee out of a possible 100, Circle YES or NO to indicate whether your opposition passports have been checked? Half time and Full time scores, Non appearance of players entered on the team-sheet should be clearly deleted by putting a line through their name. ALL PLAYERS NAMED ON THE TEAM SHEET MUST PLAY, Enter all player registration numbers in the boxes provided. Enter all goal scorers and indicate whether they have played in the match? The manager or anyone acting on behalf of the manager in their absence should print and sign their name at the bottom of the sheet.

The home team manager should collect their opponents sheet at the end of the match, staple both halves together and post to me, to arrive no later than the Thursday after the Sunday fixture having taken place.

If all of the above is not adhered to, then statutory league fines will be applied!
